2019 Beach WalkWaiver of Liability
By my signature, I accept this Waiver of Liability and Assumption of Risk in consideration of the opportunity to participate in the 2019 Beach Walk fundraiser presented by Boys & Girls Clubs of Long Beach on October 19, 2019.
I acknowledge and understand that there are dangers and risks inherent with walking/ jogging/running and agree to assume all risk of personal injury. I certify that I am physically fit to participate in the BGCLB Beach Walk.
I, hereby hold harmless Boys & Girls Clubs of Long Beach, their respective officers, directors, employees, and volunteers for all liability, for risks known or unknown.
I understand that this Waiver of Liability and Assumption of Risk is a contract and that I have read it thoroughly, understand it, and agree to its terms. By signing below, I agree to the contract terms and to pay for all medical costs, attorney’s fees, and all other costs and damages from injury to myself ward resulting from participation in the BGCLB Beach Walk.
Further, I grant full permission to Boys & Girls Clubs of Long Beach to use any photographs, videotapes, motion pictures, recordings, or any other record of this event for any purpose.
Minors accepted only with a parent or guardian’s signature.
